HMH – Primary Care – Jersey Shore Medical Associates has proudly served the Monmouth County community for more than 60 years. Established in 1961 and known locally as Jersey Shore Medical Associates, our practice is built on a commitment to patient-centered, individualized care and long-term relationships that support overall health — not just isolated concerns.

Our practice is recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home, reflecting our focus on coordinated, accessible care that emphasizes communication, continuity, and patient involvement. Our board-certified physicians and ANCC-certified nurse practitioners provide comprehensive primary care, including preventive services, well visits, same-day sick appointments, and management of acute and chronic conditions. Many of our providers have also received national recognition from NCQA for excellence in diabetes and heart and stroke care.
